1. How to Verify Your Record

1. Go to the Verify by CNIC page.
2. Enter your 13-digit CNIC or B-Form Number (with or without dashes).
3. Verify your personal details (Full Name, Father's Name, Application Number, Merit Rank).
4. Cross-check your Matriculation (10% weightage), Intermediate (40% weightage), and MDCAT (50% weightage) scores.
5. Check your Domicile District and PRC Form C status.
6. If all records match your documents, click "VERIFIED (Confirm My Details)". A digital timestamp will be permanently logged.

2. If a Discrepancy is Found (Claim Procedure)

1. Click the Claim Form button to open the Data Verification Claim Form.
2. Fill in the required fields, select the relevant discrepancies, enter the correct information, and provide a brief justification.
3. Review the completed form and click Download Claim Form to save it as a PDF.
4. Prepare clear, legible scanned copies of supporting documents, such as Mark Sheets, MDCAT Result Card, and Domicile/PRC, as applicable.
5. Email the downloaded Claim Form PDF along with the scanned supporting documents to admissions@jsmu.edu.pk.
6. Use the following email subject: Data Verification - [App No] - [Candidate Name] - [CNIC].
